As stated in the Tenant Information section of the web site, anyone who wishes to be accepted for housing or placed on our waiting list must meet specific criteria and complete the appropriate applications for their housing needs. Applications must be submitted, reviewed and approved prior to moving to the Pythian Home Retirement Center.
All applicants must complete a Reference Check Inquiry and Consent form and a Authorization and Consent for Criminal Records Investigation form. The information needed to complete these forms (and other forms for specific applications) is listed below. Please take a few minutes to review the information contained in the applications you would like to submit. When you have the documentation to complete the forms, contact Lori at 360-696-4375 or e-mail her at lori@koprc.com, and she will be glad to mail or fax you the forms you wish to complete. If you have a printer available, you may also print the forms from this web site.
Tenants may have common household pets in their units. KOPRC does not discriminate against applicants or tenants based on their ownership of a pet. This section (at the end of the web page) will also provide information on the pet policy, application, registration and rules, if you choose to have a small pet in your unit.
